Which forms do I need?
Please read all of the information below if you wish to download
forms from this site to open an account or become a CDF Online member.

Opening A New Account
If you wish to open a new account with the CDF please click here
"New Account Application Form" and
print off the attached form. Once you have filled in the form mail
it to us with a cheque and we will open the appropriate account
for you. Please remember to fill in your tax file number details.
N.B. Once your new account has been recieved at the CDF we will contact you to arrange suitable identification.
If you do not wish to send a cheque with your application form,
you can click on the "Direct
Debit Form" and print this off. Simply complete the necessary
details and send it in to us with the completed "New Account Application
Form". We will then open your account and debit your initial deposit
from your account at the financial institution you have nominated
on the Direct Debit Form.
We will issue you with a receipt no matter which deposit alternative
(cheque or direct debit) you choose, along with anything else you
need (e.g. an account transaction card) depending on what type of
account you open.

Linking your CDF Account to CDF Online

If you wish to have your existing (or new) account linked to
CDF Online, you will need to print
off the "Online Transaction Application
Form" by clicking on the link. This is a three page document.
You should read the terms and conditions on the third page before
filling in the forms. You will need to fill in the first page
and the top part of the second page. If you wish to nominate
someone else to have access to your CDF account (either full
access, i.e. the ability to make value transactions on your
CDF account or enquiry only, i.e. the ability to simply view
your account but not transact on it) you will need to have them
fill in their details on the second page of this document in
the appropriate section.

If you will be using CDF Online to transfer funds from your
account at a bank, building society or credit union you
will need to print off the "Direct Debit Form". We need to load your account
details at your financial institutions in order for you to
debit them through CDF Online. This is a two page document and
we ask you to read the second page before filling in the form.

If you think you might need to transfer funds from your CDF
account to your external accounts to a value higher than $2000
per transaction you will also need to complete the "CDF Online Variation of Daily Limits Form".
CDF Online and your Password
When you log on to CDF Online for the first time you will choose
your own password. In the event that you forget your password one
option is to phone the CDF in order to get another one and therefore
it is necessary for you to fill in the "CDF Online Security Questions Form" now. Simply print off the "CDF Online
Security Questions Form" and fill in the appropriate
details. The form requires you to nominate two security questions
yourself as well as filling in your mother's maiden name. A CDF
staff member will ask you the three security questions when you
phone to acquire a new password. This ensures the integrity and
security of your online account.
Another method of obtaining a new password is to simply print off
the "CDF Online New Password
Form" and fax it to us on 02 4979 1169.
Getting your completed CDF Online forms to the CDF
Once you have completed the appropriate CDF Online forms you
simply need to send them to the CDF via:

have received the correctly filled in forms we will phone you and
issue you with a password and you will be ready to go online!
